This is how we have been playing so I may be wrong.1) The second is ignored since you already increased the sorcerer to two you can't do it twice. The same goes for the flesh rippers getting the orb twice only increases the health once.
2)If a hero loses a quest he stays in the same hex... unless it says on the card that he/she must retreat.
3) During Setup you remove all quest cards but the ones with A in them (2A,3A,4A,etc). Then you deal them out... its impossible to get duplicates. (know that one for sure)
