I'm only 95% sure on this one but they would get the bonus again.

The hero has to leave the hex. I believe it also has to be a connecting friendly hex.

This ones easy. When setting up the board remove all the setup quest and only deal two setup quest to each player. There will be no duplication of map pieces. Also then remove all quest cards from the game that do not correspond with the map pieces in play
