Yes, it's a very strong weapon.

Each area can have a token from each player, in theory; one player's activation tokens do not affect any other players.

3. We played 2 player game and started with 2 dragon and 2 false runes on the map. It seemed really tough to get more dragon runes to get on the board, and yet we were trying to obtain and control 6? Did we miss something? How are more runes usually placed on the map?

Generally, more are placed through season cards and by questing. After a couple plays, the flow of such things becomes a bit more apparent.

4. Quest cards seemed really hard to get also. We started game with 2 each, but is the only way to draw more is by obtaining a city and using the ability to draw from it? Also rewards cards seemed somewhat rare to get, but easier than Quest cards?

Whenever you gain a reward from a quest, you should immediately draw a new Quest card. (This is in the rulebook). Basically, you should never have less than 2 quests in your hand at any given time (and if you use the city option, you could have 3).

5. If I am defending with a neutral Dragon, does the Dragon only get to draw ONE fate card? It seems when allied with neutral units the card makes their abilities easy to understand, but when drawing as the "defender" does the card also just turn into the abilities they can use?

Each unit only draws one card. A single dragon on the board isn't going to be super hard to defeat, but can still cause a lot of damage. Neutrals on the map are more speed bumps than nasty army-decimating entities. Neutrals become pretty potent when allied, however, because their abilities are pretty strong, and you can protect them with other more expendable units.

6. Heroes can basically go anywhere and not be attacked unless specifically "dueled" by another hero? Also with only 4 rounds per duel with another hero, do Heroes generally NOT defeat another hero? It seemed like a couple of misses on fate cards and the Heroes survived.

Correct, heroes can act independently, and it's not uncommon for a duel to leave both heroes alive (sometimes it takes a couple heroes to slay an enemy hero, if you really want their rewards).

7. Normal Combat. Is it true that you go through the initiative track on player boards 1-5 only ONCE, then determine "standing strength" of remaining units to determine a winner? This doesn't seem like a whole lot of fighting back and forth?? Also it seems very different to the duels?

This is correct. Knowing how to plan your army for this is critical.
